Looks like truck camper tie down points attached at the front of the bed there. That style of tie down puts a lot of stress on bed mounts

As a Michigan truck owner I can honestly say this is a common sight. It's most likely the bed not the frame. I have an 03 F250 that was a plow and salter. It's still rollin'. Who needs floor boards?
My 06 GMC Dually had a bit of that going on. What killed it was when they put it on the lift it bowed the other way and the passenger side frame broke.

Overloading isn't a problem for the beefed up suspension but it breaks the back of the vehicle so much the belly is scraping the floor!

The beds rot around the edges on these. That's a rust belt work truck. Frame is probably solid. Bed and can floor around mounts are probably the problem.
