At one time had 84 GMC stepside pick up that came with a manual tranny which I converted to automatic tranny with a external cooler just in case this ever turned into a milkshake mixer.
That happend to my friend's Fiat Stilo with a turbodiesel engine (1.9 JTD 115HP). Those engines mostly have some kind of oil cooler, in this case it was just an oil-coolant heat exchanger (some have auxillary radiators). And guess what, that exchanger developed an internal leak at some point, causing the fluids to mix. We were scared as hell that it was a head gasket failure (which in case of this engine is very rare), cause it presented exactly the same symptoms.
Example of why checking fluids regularly is a good idea, you can catch leaks when they start… that most likely didn't happen in a week and the owner would have seen signs beforehand.
